An annoyance: No SOI change marker sometimes

It just happened again.  I'm heading for Minmus, MechJeb thinks my trajectory is perfect.  There's no mark for the SOI change, though.  MechJeb's warp-to-SOI-transition will go right through it.  Simply using warp will not drop out when it does the transition.  Yet the intercept does exist, I was 3km off from what MechJeb said was the point of closest approach.

It's pseudo-random, any given flight I have no idea if it's going to fail but if I revert a flight the new one will normally behave the same as the old one.

Very known, and by many (for example me) very hated.

It's also very hard to fix. From what I've read about it, I don't think I could fix it.

I found that if your trajectory intercept another body later it will not show your first SOI change. 
This is common going to Minmus as you will cross Mun orbit twice, best solution is probably to do an small burn to miss the mun intercept then set an new node for Minmus intercept and note the time you will enter Minmus SOI, reduce warp to 10 or 50 at this time and control the vessel at this time, you will change to Minmus SOI as predicted and can continue as normal.

This is more dangerous around Jool as you can easy get Tylo Intercepts going to Vall from the outer moons and also Vall intercepts going to Laythe. Worst case this can send you into Jool or out in solar orbit. Watch the map then you do burns as the intercept will show up then disappear as you get an target soi intercept.

Aha!  It was showing a SOI intercept on Mun, although on the third crossing of the Mun's orbit.  I'll keep my eyes open, that very well might be the whole bug.  (Showing the last SOI intercept rather than the first or all.)
