Craft Dissapeared While in Low Orbit of the Mun (while on rails)

I had a vessel in fairly low orbit of the mun (5km to 6km).  Although this is lower than the highest point on the mun, I watched it make a full orbit before leaving it.  I then sent another craft to rendezvous with it, but as I switched between map view, it suddenly stopped appearing on the map.  Unable to complete the maneuver (as I had no target), I reverted to launch and watched the craft orbit the moon fully again without crashing.  Is this a bug?  Is there a point where a craft that's on rails will crash by default?

as long as the orbit is not 100% equatorial, the surface it is passing will change with each orbit, so just one orbit might not be enough to be sure....

u could try to timewarp around 60 days and watch it surviving (or fail while trying :D)
