Mechjeb clean install missing several information windows in KSP 1.12.1

As the title says, Mechjeb is missing several windows for me in 1.12.1 and I don't know what's going wrong. Notably, the delta-v, orbital info, surface info, and vessel info tabs do not appear to exist in the Mechjeb menu at all. Additionally, the settings tab is broken, showing up as a tiny unclickable box in the top left corner when I try to open it. I have tried downloading the latest version of Mechjeb via github, and encountered this problem. I have also tried copying forward a version that worked fine in 1.11, but the issue still persists in 1.12.2. Does Mechjeb have any particular dependencies that I don't know of? I know it needs Module Manager but other than that is there anything that could be causing this?

I encountered the same issue when I updated to the newest versions of Toolbar and Toolbar Controller. Go back a version on both of those and it should work properly again.
