Extraction vessels keep vanishing?

Twice now I've sent a vessel to the Mun to extract ore to fulfill a contract, and both times after switching to the survey scanner to find a spot to land, the ship just disappears. What's up with that? It was in a stable orbit around the Mun both times.

Was it in a really really low orbit?

Did it have a probe core?

Is your Persistent Debris setting set to 0?

maybe the game read it as a relay probe, and therefore hid it? check if there are hidden categories in the map view, and if your probe may fall under them

They were piloted craft, orbit was ~ 30 km.

try to go in the astronaut complex then. what happened to the astronauts?
