Finally ready to bring my Interplanetary crew transport back from its mission to Duna (where it was deploying comm satellites). Start the ejection burn, watching it in map view,

...Oh yeah nuclear engines overheat when grouped with the quad coupler... right...

That was a quickload.

Sending a rescue mission to pick up my stranded Kerbonaut from the Mun in a one-person capsule.

"Hi Bill! Rescue? Oh no, I've no room to rescue you. Well, bye!"

Just done a 6 day mission to the moon, 1 day for the launch, transfer, landing, escape and return.

5 days of aerobraking cos I ran out of fuel...

payload testing my new rocket series and forgetting to engage SAS on liftoff.

After getting my space station in orbit for a test run, I accidentally left the last module's service engine attached. Mistakenly hit shift and watched the whole thing enter a unfixable deorbit.

Accidentally pressing SHIFT (particularly in map view) is the most common reason for me to reload a quicksave.

I should remap it, but then I would end up perpetually confused.

I have a new one, courtesy of Deadly Reentry: Not putting my parachutes high up enough on the capsule that they're fully shielded by the pod.

Fortunately, redundancy saves the day again.

Oh, any time I've even considered playing KSP while drunk, that's ended in numerous facepalm moments. Most notable was when I carefully left my Minmus lander in a stable orbit before heading to the pub, and woke up the next morning to find it scattered all over the landscape, and only a vague recollection of what had happened!

Last weekend I fell asleep while flying a plane across the ocean (I was kinda tired..). I wake up to the sight of the air intakes on the shore, 1/4th the way around kerbin.
