That would be Apollo 9 - David Scott and Rusty Schweickart did the EVAs in the first spacesuits with independent life support. It was also the first time that American spacecraft were given names by the crew (the CM was Gumdrop and the LEM was Spider), the first docking of two spacecraft with crew transfer (Scott was also aboard Gemini VIII with Armstrong, the first docking in orbit). Lots of firsts on that mission. (Scott was also the first man to drive on the Moon).

According to Gene Krantz in "Failure is not an Option: Mission Control from Mercury to Apollo 13 and Beyond" on page 84 the Mercury spacecraft were named by the astronauts.

The Mercury spacecraft were given names by the astronauts but the Mercury-Atlas or Mercury-Redstone designation was used by the launch and flight teams. The word "flight" refereed to the spacecraft and booster events from liftoff to landing in the Mercury program. During the Gemini and Apollo programs, the term "mission" was used by the program, launch, and mission teams. "mission" carried the collective connotation needed when dealing with two or more spacecraft or launch vehicles. Although this was the standard, the terms "mission" and "flight" were often used interchangeably.

This is one gripe I have about those IVAs. It does not seem practical to depressurise the whole chamber.

Well, maybe the walls are thick, and there is just enough room between inner and outer doors for a little kerbal to squeeze in.

Or maybe they just went with depressurising the whole cabin to create more space for snack storage.

I think the spacesuits for the Kerbals are just exoskeletons. Kerbals might just be bean-bags with smiley faces.
