i have this upon launch. Before the update, i didnt have any problems. But its difficult to do the missions without it. I did figure out that if i dont jettison my boosters prior to reaching orbit, this doesnt happen

After almost 80 hrs of gameplay, I finally made it to Duna. Unfortunately my craft hit too hard. Those Kerbalnauts will be remembered for their sacrifice.

It happens on my older laptop but not my new desktop. Could be a hardware issue.

Same for the Mun, and then after walking away from the ship to explore since he was stranded, the ship mysteriously disintegrated in the distance. That kerbol is now stuck looking at the destruction and knowing he has no way home.

I had the same issue while on EVA on the Mun. Pressing F to grab and B to board did nothing even tho the option was available on my screen.
