Is there a bug related to de-couplers not decoupling?

Hiya,

So, I have two SRB's on some basic staging. Like literally a fresh save. Each SRB is attached via a tt-38k decoupler. But when I cycle through the staging, it doesnt decouple. It makes the noise, and the tt-38k piece falls off, but the SRBs are still attached. Ive tried doing it with slow velocity, fast velocity. Stage with another rocket kicking on, stage without a rocket kicking on. Ive tred 3 decouplers PER SRB. With or without spacetape.

It's possible to accidentally attach the boosters directly to the central fuel tank even though they look like they're attached to the decouplers. In the VAB, try removing those and very carefully re-position them on the decouplers. 

This might not actually be the cause of your problem but it is a very common thing to have happen.

I certainly tried moving them all around. Tried in sandbox. Tried having them closer, further. Tried more couplers, less. I just dont know

In the VAB, try clicking to pick up the decoupler and see if the booster comes with it.

When placing the SRB make sure your mouse cursor is over the decoupler. The SRB will attach to whatever your mouse is on when you click.

A suggestion: see if any errors pop up in game console (Shift-F12) when you try to decouple.
