A reliable launch vehicle decided to just skip a stage and left a Mun base component stuck in Kerbin orbit with not enough dV to make it to the mun, trailed by fully fueled up 2nd stage attached to a burned out 1st stage.

I accidentally blew up my spaceplane on Minmus. Which sucks. I was attempting to reach orbital speed while travelling across the surface and I lost control and killed the pilot. RIP in peace.

Had 3 kerbals orbiting the Mun when a glitch removed all but their capsule. Sent a rescue and then decided to land, only by then the base had passed onto the nightime side of the Mun. Landing on uneven terrain caused me to crash, but the capsule survived and the crew made it to the base. When morning came two of the three went for a drive in a rover, crashed and were killed. Not a lucky mission that one...

Trying to bring an expedition to the south pole home via aircraft. Since the aptly named aircraft "Fat Walrus" needs a bump or slight drop to lift off (like the runway end) and the polar ice is incredibly flat, i decided to use the drop from the polar ice shelf into the ocean to go airborne. The result: 3 dead Kerbals, some explosions, and lots of expensive equipment sunk into the cold water. Apparently there are some invisible holes in the ground right before the cliff.

So today I finally got into Duna, using an Apollo-style rocket. Everything went smoothly (remembered to retract the panels before aerobraking and I didn't intercept Ike), until I undocked the lander and fired my engines retrograde. I then found out why people use RCS to move away from the mothership before firing the engines:

Then I quickloaded and attempted a safer landing. Turns out that I used 3-way simmetry with the landing legs and 4-way simmetry on the engines, which made the lander spin out of control. Bill and Bob did their best, but the lander span out of control and "lithobraked" too hard:

Don't break moons, its an limited number of them, anyway an insane picture.

This is rather the opposite, an test of aerobrake with deadly reentry and an dockable heat shield, planned to use the miner in the Jool system and wanted to test it aerobraking at Laythe after mining on Pol.

It did not work well.

So, today's Minmus landing was a fiasco.

First, timewarp messes up my encounter. Instead of the carefully-arranged low periapsis, I get one really far out, and have to spend about 200 m/s on the insertion burn, plus 50 on the circularisation.

Then, I do the usual on landing,

Next, I have a go at righting the lander. Taking a run-it with the jetpack and crashing into the legs, I nearly get it onto its feet,

But it falls over again. So I go faster. This just tips it so I can't even reach the hatch any more.

I nudge it over so I can get to the hatch, and decide to just screw it and fly off anyway. Hey, it worked on the Mun! Well, it didn't work on Minmus. One of the legs smashes off, unbalancing the lander and sending it spinning. I kill the engine, get it under control, and resume burning prograde.

Remember that issue with the insertion burn that cost me a bunch of dV? Well, this was a direct ascent lander, so now's when I run out of fuel, nowhere near orbit. I knew I was short, but hoped to just about make it. I could ditch the capsule and EVA, but I want to bring back all the Goo canisters, not just one result. So I decouple the capsule, get out, and try and push it to orbit.

Finally, having foolishly spent all my EVA propellant, there's nothing to do but say a few last worst and wait for impact.

After launching sometime in February, the lander Ice Grouse finally arrived at Eeloo today. What happened next was a cluster****.

I didn't expect the planet to rotate that quickly, so correction burns to keep the landing zone steady ate into the already limited delta-v budget. The lander ran out of fuel before coming to a halt and smashed into the ground at about 25 m/s, sideways. Amazingly, it only lost one of the exposed engine pods, and I could right it after a long tumble. Then the rover was decoupled. What's wrong with that, you say?

See, the lander carries two Mystery gooTM canisters full of not-yet-transmitted science. Also: no antennae to be seen. That's because there aren't any! And since the rover can't redock, well... seems that I lugged 300 kg to the a** end of their system for nothing.

It's a pretty common error, especially if you're used to the Mun where it doesn't matter, or even Kerbin where you can jump higher.

Though I will admit, it doesn't look like that lander would get back off Eve anyway.
