This is what happens when you forget about setting up your final stages in your excitement to get to the Mun and back. On departure I activated what I thought was the next stage which would separate the return craft from the landing stage. Instead the entire ship came apart all at once, stranding the brave astronauts on the Mun.

And in a similar vein, I set a rover in to orbit of the Mun, also with a staging configuration failure. lol.

I've since learned that adding and removing parts in the builder can drastically modify staging setups.

Behold, the Duna Explorer. An orbitally assembled mother ship designed to fly a crew of 12 to Duna and Ike, do some science, refuel from mined Kethane, and return to Kerbin.

After painstakingly testing (almost) every part of the Duna Explorer, including the Kethane Mining Rig, sensor probe, landers, ballute aerobrake system, I finally luanched to Duna and Ike.

One thing I did not test, was deployment of the communications dishes (using RemoteTech2). Since, I run FAR, I tucked that folding antenna behind a bit of fuselage...

Now, that we're on our way, lets deploy it.

Not too big, clips a little bit. Shouldn't be a problem, it's flexible.

Wait, is that another fold out? It gets bigger?!

Is that dish slicing into my ship?

That's not good.

Mistakes were made.

Why are you using that dish for Duna, anyway? The little stock dish has more than enough range, weighs about 1% as much, and requires far less space. That monster is only needed for missions to Jool or beyond.

My biggest fail was probably the core of my space station. Somebody (me) forgot to make calculations. The core was composed by command cupola and Hitchhiker storage container, plus some scientific equipment, battery, mechjeb and solar panels. Since it was very heavy, i thought my old 0.16 Ker-boom 5 will not handle it. I decided to make a new rocket. It was using the new quadra-coupler. Propulsion was composed of 8x Rockomax BACC , 4x LV-30 engine and finally 4x LV-N.

Everything was going well until circularization. I didn't realise that four LV-N engines are simply not enough to pull my ship onto the orbit.

P.S. I managed to circularize Jeb by his pack, but bob and bill are probably in heaven now :-/
