Was that amountain I just crashed into?

More than a few times I'm happily into my munar orbit descent burn and I'm down to 3-4 km from the surface (or so I'm led to believe) when suddenly, KABOOM! i explode into a million bits. Did I hit a mountain and if so, why didn't this show up in the altitude to surface?

I might add, if I didn't hit a mountain, what the devil happened? I didn't make any control changes.

If it was KSP1, when you approach the Munar surface at altitudes of less than 7,000 meters, click on the ocean icon to the left of your altimeter. That changes it to terrain altitude. Some terrain on the mun reaches 6,000 meters and the average elevation is about 2 kilometres near the equator. Remember that you may not stop in time!
